MGSV: Phantom Pain Side Surgery
In-game side-ops are short missions that mainly involve extracting a target, finding specific objects, or practicing aiming.
A side operation can be completed in a number of ways as there is no grading. It is better to complete these missions alongside the main one, as some of them have certain requirements that need to be met.
In this guide, we will cover all of the side operations in the game along with their brief guide:
#1 – Extract Interpreter – Russian
Location: southern Afghanistan
Voranf: Mission-2
Go east of Spugmay Keep and fulton the guy in a green beret to complete this mission. You can also tag the soldiers from a nearby hill to make the mission a little easier.
#2 – Extract interpreter – Pashtu
Location: southern Afghanistan
Voranf: Lateral-27
You must go to the Moutain Relay Base and fulton the objective to the Mother Base to complete the mission.
#3 – Extract interpreter – African
Location: North Africa
Voranf: Mission-14
You must go to the west of the Bampeve Plantation and silence all the enemies, followed by the extraction of the main objective to complete the mission.
#4 – Extract Interpreter – Kikongo
Location: Southeast Africa
Voranf: Mission-18
You must go to the Munoko ya Nioka station and extract the primary objective to complete the mission.
Collecting Blueprints missions
In these missions you have to collect blueprints for different weapons and tools:

#11 – Extract Highly Skilled Soldier #1
Location: western Afghanistan
Voranf: Mission-2
To complete this mission you need to fulton a sniper located on the south side of the Yakho Oboo Outpost along with a couple of guards. Approach the area from the west side to complete the mission with ease.
#12 – Extract Highly Skilled Soldier #2
Location: southern Afghanistan
Voranf: Page-Up-5 und Page-Up-11
The soldier you need to fulton is on the east side of Da Wialo Kallai. Approach the objective from the east side to complete the mission with relative ease.
#13 – Extract Highly Skilled Soldier #3
Location: Eastern Afghanistan
Voranf: Mission-6 and Side-Op-12
You have to go to the heart of Da Smasei Laman to find your target along with some other villains. Approach them one by one to complete the mission without difficulty.
#14 – Extract Highly Skilled Soldier #4
Location: Central Afghanistan
Voranf: Mission-7 and Side-Op-13
You have to go to Da Ghwandai Khar to find your target with some other enemies. Approach the area from the east side and lure the target to your position to slightly fultonate them to your position.
#15 – Extract Highly Skilled Soldier #5
Location: Northwest Afghanistan
Voranf: Mission-8 and Side-Op-14
Your objective is in the southern part of the Sentry Post, east of the ruins of Aabe Shifap. You need to approach the hill from the south side to easily fultonize the soldier.
#16 – Extract Highly Skilled Soldier #6
Location: Southeast Afghanistan
Voranf: Mission-13 and Side-Op-15
You'll find your objective on a small outpost between Easter Communications Post and Mountain Relay Base. Eliminate enemies one by one and you won't have any trouble getting closer to your goal.
#17 – Extract Highly Skilled Soldier #7
Location: Southeast Africa
Voranf: Mission-14
You will find your destination on the west side of Munoko ya Nioka station. You should approach the target from the west side; Pin down all enemies in your path and fultonize the target.
#18 – Extract Highly Skilled Soldier #8
Location: Northwest Africa
Voranf: Mission-16 and Side-Op-17
You will find your destination at the Mfinda Oilfield; on the plain beneath the oily water. You should approach the area from the west side, climb some fences and drop down to find your objective.
#19 – Extract Highly Skilled Soldier #9
Location: Central Africa
Voranf: Mission-19 and Side-Op-18
You will find your destination northwest of Ditadi Abandoned Village. I recommend approaching the area from the north side and luring the target to your position instead of going straight to it.
#20 – Extract Highly Skilled Soldier #10
Location: North Africa
Voranf: Mission-18 and Side-Op-19
You will find your objective on the south side of Bampeve Plantation. There are a few guards patrolling the area, but dealing with them isn't very difficult. Deal with the guards and fultonize the target to complete the mission.
#21 – Extract Highly Skilled Soldier #11
Location: Southeast Africa
Voranf: Mission-20 and Side-Op-20
You can find this enemy in the Munoko ya Nioka station with a small army of enemies. Reach the enemies one by one before completing the primary objective.
#22 – Extract Highly Skilled Soldier #12
Location: Northeast Africa
Voranf: Mission-23 and Side-Op-21
You need to fulton a sniper on the south side of the Kungenga Mine. The main objective is along with some other snipers, so it is better to approach the target while crouching. Incapacitate targets one by one with a non-lethal sniper rifle, then hit the primary target.
#23 – Extract Highly Skilled Soldier #13
Location: southern Afghanistan
Voranf: Mission-31 and Side-Op-16
Your target is a sniper who is on the west side of Spugmay Keep along with another sniper. The idea is to reach the area from the north of the sniper and use a non-lethal sniper rifle to incapacitate the targets. mission completion.
#24 – Extract Highly Skilled Soldier #14
Location: Northern Afghanistan
Voranf: Mission-31 and Side-Op-23
You can find this enemy on the east side of the central base camp of Afghanistan; near a shooting range. Reach the objective via the stairs in the south of the area and take it to Mother Base.
#25 – Extract Highly Skilled Soldier #15
Location: East Africa
Voranf: Mission-31 and Side-Op-22
You need to fulton the guy patrolling the top floor of the Lufwa Valley Mansion. The best way to complete the mission is to approach the mansion from the front entrance, getting rid of enemies along the way before Fulton takes him to the Mother Base.
#26 – Extract Highly Skilled Soldier #16
Location: Northeast Afghanistan
Voranf: Mission-31 and Side-Op-24
To complete the mission, you must infiltrate OKB Zero and extract an enemy from under the arcades. in the heart of the facility. There are many resources in the area for Mother Base, so take your time with the mission and then extract the objective.
#27 – Prisoner Extraction #1
Location: Southeast Afghanistan

Go to the middle of the Eastern Communications Post and find the prisoner in the building with the radio transmitter. Reach the area from the west side and fulton the prisoner to Mother Base.

Location: Southwest Afghanistan

You will find the prisoner you must rescue in Shago Village; on the top floor of a building. To make things a little easier, consider tagging as many enemies as possible.
#29 – Prisoner Extraction #3
Location: Northern Afghanistan
Voranf:Mission-8 and Side Ops-28
The prisoner is at Afghanistan Central Base Camp. You should take the similar route as in Mission-12 by going to the easternmost hangar instead of the one on the north side to reach the prisoner.
#30 – Prisoner Extraction #4
Location: Southeast Afghanistan
Voranf:Mission-9 and Side Ops-29
This prisoner is on the west side of the bridge near Mountain Relay Base. Reach the area from the northeast side and tag all enemies along the way to complete the mission without difficulty.
#31 – Prisoner Extraction #5
Location: Eastern Afghanistan
Voranf:Mission-10 and Side Ops-30
The prisoner you need to rescue is in the sentry post on the west side of Da Smasei Laman. Disable all soldiers in the area before moving towards the prisoner.
#32 – Prisoner Extraction #6
Location: Central Africa
Voranf: Mission-13
The prisoner you are looking for is in a small sentry post on the western side of the Abandoned Village. Before you reach the prisoner, you need to take care of the sniper that is on the west side of the sentry post.
#33 – Prisoner Extraction #7
Location: Central Afghanistan
Voranf:Mission-13 and Side Ops-31
The prisoner you need to rescue is on the hill on the east side of Spugmay Keep. You will have no such difficulty in accessing the prisoner.
#34 – Prisoner Extraction #8
Location: North Africa
Voranf:Mission-16 and Side Ops-32
You will find your destination in the heart of Bampeve Plantation. It is highly recommended that you silence any guards in the area before Fulton takes them - including the prisoner - to the mother base.
#35 – Prisoner Extraction #9
Location: Northwest Afghanistan
Voranf: Mission-17 and Side-Op-33
This prisoner is in an empty pool in the ruins of Aabe Shifap, surrounded by four snipers in different locations. I highly recommend taking care of all those snipers before actually going in to rescue the prisoner.
#36 – Prisoner Extraction #10
Location: western Afghanistan
Voranf: Mission-13
You can find the prisoner in the Khaate Palace. I recommend accessing the area from the north side to complete the mission without much difficulty.
#37 – Prisoner Extraction #11
Location: Northwest Africa
Voranf: Mission-18 and Side-Op-34
Head southeast of the Bwala ya Masa and you'll find the prisoner you're looking for on a rooftop. Don't forget to look out for some wild animals roaming the area.
#38 – Prisoner Extraction #12
Location: East Africa
Voranf: Mission-20 and Side-Op-37
This prisoner is in the industrial zone in front of the tunnel. The idea is to take care of all the snipers in the area before heading out to rescue the prisoner.
#39 – Prisoner Extraction #13
Location: North Africa
Voranf: Mission-20 and Side-Op-38
You need to go to the south side of Bampeve Plantation and go down the hill from the northwest side to rescue the prisoner.
#40 – Prisoner Extraction #14
Location: Northwest Africa
Voranf: Mission-21 and Side-Op-39
You must reach the ravine near the south side of the Kungenga Mine to find the prisoner you are looking for.
#41 – Prisoner Extraction #15
Location: South West Africa
Voranf: Lateral-40
You need to go to the southern hangar at Nova Braga Airport to find the prisoner inside. Don't forget to take care of the nearby sniper as he can sound the alarm once he spots you.
#42 – Prisoner Extraction #16
Location: Southeast Africa
Voranf: Mission-26 and Side-Op-41
You need to go to the west side of Munoko ya Nioka station and find the prisoner in a small building. Reach the area from the north side and deal with a few guards before rescuing the prisoner.
#43 – Prisoner Extraction #17
Location: Northeast Africa
Voranf: Mission-26 and Side-Op-42
The prisoner is in a small building on the east side of the Kungenga Mine. Make sure you lure the guards away and deal with the snipers patrolling the area before heading out to rescue the prisoner.
#44 – Prisoner Extraction #18
Location: Central Afghanistan
Voranf: Mission-31 and Side-Op-36
You can find the prisoner in a house in Sakhra Ee. It is highly recommended that you stick to a stealthy approach during this rescue mission, otherwise it will become quite difficult.
#45 – Prisoner Extraction #19
Location: Northeast Afghanistan
Voranf: Mission-32 and Side-Op-44
You must travel to OKB Zero and find the prisoner in a building near the west wall of the base.
#46 – Prisoner Extraction #20
Location: Southeast Africa
Voranf: Mission-35 and Side-Op-43
You can find the prisoner in a building in the heart of the Munoko ya Nioka station. Enter the area from the west side, always approaching stealthily to avoid being spotted by the gunship. Fulton's primary goal and go as fast as possible.
#47 - Lost Sheep
Go to the northeast side of the village and deal with the soldiers before using Fulton on the lost sheep. You only have to deal with a few soldiers.
#48 – Legendary Brown Bear
Head to the southeastern ruins of Aabe Shifap and use non-lethal weapons to calm the beast before taking it to Mother Base with a Fulton.
#49 – Legendary Ibis
Head towards the large waterfall in Lufwa Valley and use binoculars to identify the bird. You can use a non-lethal sniper rifle to calm the bird and send it to MB.
#50 – Legendary Jackal
You can find the animal you need to find in the savannah south of Nova Braga Airport with a sizable pack.
#51 – Wandering Mother Base Soldier #4
You will find the destination west of Nova Braga Airport. While you're at it, make sure you don't get spotted.
#52 – Wandering Mother Base Soldier #5
You can find this soldier near the Kungenga Mine with a shield on his back. He will use a flashbang on you when he spots you.
#53 – Wandering Mother Base Soldier #6
You can find this soldier east of the Munoko ya Nioka station. If he uses his cloaking device, make sure you equip your night vision goggles.
#53 – Wandering Mother Base Soldier #7
You can find this soldier south of Yakho Oboo Outpost, under the bridge. Equip your night vision goggles when using a smoke grenade or stealth ability.
#54 – Wandering Mother Base Soldier #8
You can find this soldier on the south side of the Lufwa Valley. He will use a sleeping grenade if he manages to spot you.
#55 – Wandering Mother Base Soldier #9
You can find this soldier on a hill east of Sakhra Ee. He'll drop a stun grenade when he spots you.
#56 – Wandering Mother Base Soldier #10
You will find this soldier in the ruins of Aabe Shifap equipped with a shield that makes this extraction a bit difficult.
#57 – Unlucky Dog #1
You can find this prisoner in a cell on the north side of Yakho Oboo Outpost. You can take any approach you see fit and fulton it to Mother Base.
#58 – Unlucky Dog #2
You can find these prisoners in a small area near the steps of the amphitheater in Da Smasei Laman. Silence the guards and silence the prisoners to complete the mission.
#59 – Unlucky Dog #3
The first prisoner is on a roof on the east side of Nova Braga Airpot and the second prisoner is on the roof of the main airport building.
#60 – Unlucky Dog #4
The first prisoner is just outside the tent on the west side of the Lufwa Valley. The second prisoner is at the base of a small waterfall near the north side of the mansion. Beware of a gunship roaming the area.
#61 – Unlucky Dog #5
Both prisoners in this side mission are in OKB Zero, so you'll need to rely on heavy camouflage to extract them to Mother Base. It is best to move along the south wall.
#62 - Eliminate Heavy Infantry #1
You must eliminate two armored guards at a sentry post north of Lamar Palace. Stealth is your best choice while completing this mission.
#63 – Eliminate Heavy Infantry #2
The infantry units you need to eliminate are located southeast of the Yakho Oboo outpost. The best idea is to grab them from afar and be done with the mission.
#64 – Eliminate Heavy Infantry #3
You must silence three armored guards individually with CQC and take them to Mother Base to complete the mission.
#65 – Eliminate the #4 Heavy Infantry
You must lure your targets into the Qarya Sakhra Ee one at a time, and then fulton them to the Mother Base. Going in with guns blazing is a bad idea as reinforcements will arrive very quickly.
#66 – Eliminate the #5 Heavy Infantry
You must eliminate four targets in this side operation. You can eliminate them one by one with a sniper rifle or a launcher. You can also incapacitate them from behind, but always start with the soldiers in the watchtower.
#67 – Eliminate the #6 Heavy Infantry
You must eliminate the group of four soldiers in the sentry post just south of Nova Braga Airport. Approach the area from the north side and deal with the patrols first.
#68 – Eliminate the #7 Heavy Infantry
This patrol is on the west side of Mountain Relay Base. Using CQC for each member is the best ideal.
#69 – Eliminate the #8 Heavy Infantry
This unit is stationed at the sentry post to the west of Da Smasei Laman. Use a sniper rifle from a distance to get closer more easily.
#70 – Eliminate the #9 Heavy Infantry
You can find these six soldiers on the east side of Bwala ya Masa. You'll find that it's relatively easy to use CQC attacks on them.
#71 – Eliminate the #10 Heavy Infantry
You can find this squad of six soldiers south of Bampeve Plantation. Approach the area from the east side to make it relatively easier.
#72 – Eliminate the #11 Heavy Infantry
This group is on the north side of the industrial zone. Use whatever approach you see fit and use CQC attacks one at a time.
#73 – Eliminate the #12 Heavy Infantry
These seven soldiers are located in the outpost northeast of the Munoko ya Mioka station.
#74 – Eliminate the #13 Heavy Infantry
This group of armored guards is located south of the abandoned village of Ditadi. You must incapacitate them with CQC and direct them to Mother Base.
#75 – Eliminate the #14 Heavy Infantry
These eight soldiers are located in the outpost just south of Bampeven Plantation. Don't use open conflict and try to lure them to your position for an easier ending.
#76 – Eliminate the #15 Heavy Infantry
You can find this group in the sentry post south of Da Smasei Laman. You should tag them from the hill on the west side and use a sniper rifle to make things a bit easier.
#77 – Eliminate Heavy Infantry #16
You can find this squad in the sentry post southeast of Mfinda Oilfield. The best idea is to approach them behind the houses in the west of the area and lure them to your position.
#78 - Get in touch with Emmerich
You have to go through the red door on the east side of the Serek Power Plant by jumping over the fence.
#79 - Demining #1
These seven mines are located southwest of Spugmay Keep.
#80 - Demining #2
These seven mines are located in a gorge northeast of Lamar Khaate Palace.
#81 – Demining #3
This minefield is located west of the Mfinda Oilfield.
#82 – Demining #4
You can find these eight mines in a ravine on the west side of Da Smasei Laman.
#83 – Demining #5
These nine mines are located northeast of the Munoko ya Nioka station.
#84 – Demining #7
You can find these 11 mines west of Qarya Sakhra; near a sentry post.
#85 - Demining #8
These 11 mines are on the hill southeast of Bampeve Plantation.
#86 – Demining #9
You can find these 12 mines in a small abandoned settlement south of Afghanistan's central base camp.
#87 – Demining #10
This minefield is located in the jungle southwest of the Lufwa Valley mansion. near the wooden bridges and the big waterfall.
#88 - Eliminate armored vehicle unit #1
You can find this unit north of the Yakho Oboo Supply Outpost. You should try to silence the soldiers before destroying the vehicles.
#89 - Eliminate Armored Vehicle Unit #3
This unit is located on the east side of the central base camp of Afghanistan. It is recommended to eliminate the snipers before approaching the vehicles.
#90 - Eliminate Armored Vehicle Unit #4
This property is on the road to the east side of Ya Masa. Once again you must deal with the sniper before destroying the vehicle.
#91 - Eliminate Armored Vehicle Unit #5
This unit is located near the west of Ditadi Abandoned Village. You have to deal with two armored vehicles and three pairs of soldiers.
#92 - Eliminate Armored Vehicle Unit #6
The unit is located near the Munoko ya Nioka station with a gunship, two armored vehicles and six soldiers. It is better to approach the area from the south side.
#93 - Eliminate Armored Vehicle Unit #7
This unit is located near the south side of Nova Braga Airport. You must approach the area from the east side and dispatch soldiers with sniper rifles before you reach the armored vehicles.
#94 - Eliminate armored vehicle unit #8
You can find this unit near the south of Da Ghwandai Khar. You must approach the area from the south side and recover the fulton armored vehicle.
#95 - Eliminate Armored Vehicle Unit #9
You can find this unit on the east side of the Kungenga Mine with a sniper rifle near the west side. You must approach from the southwest side and use the two small houses to your advantage.
#96 - Eliminate Armored Vehicle Unit #10
This unit occupies Nova Braga Airport. You'll need to approach the area from the northwest and deal with the snipers before actually recovering the vehicle.
#97 - Eliminate Armored Vehicle Unit #11
The unit is in the ruins of Aabe Shifap with a trio of snipers, making this mission a bit difficult. You must approach the area from the west side and deal with the snipers before Fulton can recover the vehicles.
#98 - Eliminate Armored Vehicle Unit #12
This unit is stationed on the outskirts of the Mfinda oil field with two heavily armored vehicles, two snipers and a few soldiers. You should approach the area from the southeast corner and sneak in with a non-lethal sniper rifle.
#99 - Eliminate Armored Vehicle Unit #13
This unit is located on the western side of the abandoned village of Ditadi.
#100 – Extract the Legendary Gunsmith
You will find the gunsmith in Bwala ya with a group of friends. You must silence the group and recover Fulton the gunsmith.
#101 – Extract the Legendary Gunsmith again
You can find the Gunsmith at Afghanistan Central Base Camp; in the south of the westernmost hangar. Deal with the sniper before turning your attention to the soldier and the target.
#102 – Extract the Legendary Gunsmith again
You can find the Gunsmith in the Yakho Oboo Supply Outpost in the same area where you found Malak in Mission-10.
#103 – Extract containers for materials
You can find the container you need for the Fulton restoration on the southwest side of Bampeve Plantation.
#104 – Visit rest
You must visit Quiet on the Mother Base Medical Platform and listen to the entire conversation to complete the mission.
#105 - Intel Agent Extraction
You can find the objective at Da Ghwandai Khar; in the same room as Miller during Mission-01.
#106 - Eliminate Tank Unit #1
You can find the tank you need to eliminate on the southeast side of Da Ghwandai Khar with a large group around the tank.
#107 - Eliminate Tank Unit #2
You'll find this unit on the east side of Lamar Khaate with a few snipers on the water; one on the north side and the other on the west side.
#108 - Eliminate Tank Unit #3
This unit is located in the ruins of Aabe Shifap with a few soldiers and a gunship. You must silence the soldiers before approaching the tank.
#109 - Eliminate Tank Unit #8
This unit is located on the east side of Munoko ya Nioka Station. The best idea to complete the mission to approach the area from the south side and silence enemies in passing.
#110 - Eliminate Tank Unit #9
You'll find this unit on the north side of the Yakho Oboo Supply Outpost with four snipers on guard. You should approach the area from the south side and silence all enemies on the way.
#111 - Eliminate Tank Unit #10
This unit is located on the west side of the abandoned village of Ditadi with two snipers, four soldiers and a gunship. You should consider approaching the area from the north and continue silencing enemies along the way.
#112 - Eliminate Tank Unit #11
This unit is southwest of Spugmay Fortress with about four soldiers and three snipers.
#113 - Eliminate Tank Unit #12
This unit is located between the Mfinda Oilfield and Bwala ya Masa with two snipers on the east and south sides. It's best to approach the area from the north side while silencing any enemies along the way.
#114 - Eliminate Tank Unit #13
You'll find this unit near the Nova Braga outpost with a few soldiers and snipers surveying the tank. The best idea is to approach the area from the west side, silencing any enemies on the way.
#115 - Eliminate Tank Unit #14
This unit is located near eastern Afghanistan Central Base Camp. You should approach the area from the south side and deal with the sniper before going inside to secure the vehicle.
#116 - Eliminate the wandering puppets #1
Infiltrate the Yakho-Oboo stash and clear each room before confronting your main objective.
#117 - Eliminate the wandering puppets #2
You can find the target you need to eliminate in the main building of Nova Braga Airport.
#118 – Eliminate the Wandering Dolls #3
This mission takes place in Da Shago Kallai with soldiers inside and outside the building.

You can find this unit on top of the hill on the northwest side of the industrial area.
#120 – Eliminate the wandering puppets #5
You need to find 8 puppet soldiers in Lamar Khaate Palace.

You need to go to the Serak Power Plant and find 8 puppet soldiers inside and outside the building.

You have to find the soldiers near the Da Ghwandai Khar, inside and outside the house where you fought at the beginning of the game.

You can find these eight soldiers in Bampeve Plantation.
#130 – Eliminate the wandering puppets #15
These eight soldiers are in the streets and houses of Bwala ya Masa. It is recommended to take a launcher with you during this mission.
#131 – Extract the Al Pod
You need to go to the easternmost hangar in Afghanistan Central Base Camp and interact with the Al Pod.
#132 - Search for the Escaped Children #1
You can find the child in the south building east of the entrance to the Mfinda Oilfield.
#133 - Search for the Escaped Children #2
You can find the child in the mansion on the east side of the Lufwa Valley.
#134 - Search for the Escaped Children #3
You can find the boy on top of one of the towers of the Aabe Shifap ruins. You have to silence the animals before you go to save the kid.
#135 - Search for the Escaped Children #4
The boy is hidden on the upper floor at the southern end of Lamar Khaate Palace. You must silence the soldiers before rescuing the child.
#136 - Search for the Escaped Children #5
You can find this boy in the Da Smasie Laman, at the east end of the upper floor of the cave network. You must deal with the soldiers before rescuing the child.
